Advanced Nurse Practitioner
Primary Care CareersWalton Medical Centre has an exciting opportunity for an advanced nurse practitioner to join our practice. This role can be offered on a full or part-time basis, with flexibility on days and hours.
The post-holder will:
- Conduct comprehensive assessments, formulate differential diagnoses, and implement evidence-based treatment plans.
- Manage patients presenting with undifferentiated and complex health needs, both acute and chronic.
- Provide expert clinical leadership and subject matter guidance in the management of long-term conditions.
- Triage and prioritise clinical needs, including urgent or emergency situations, and initiate appropriate emergency care when required.
- Request, interpret, and act upon diagnostic investigations (e.g., pathology, imaging).
- Independently prescribe and review medications in line with national guidelines and clinical protocols.
- Promote medication adherence, providing education on pharmacological and non-pharmacological treatment options.
- Perform minor procedures and wound care within the scope of competence.
- Maintain accurate, contemporaneous clinical records in accordance with legal and organisational requirements.
- Adapt communication styles to meet the needs of patients and carers, including delivering complex or sensitive information.

About the Candidate
The ideal candidate must have the following;
- Masters degree in Clinical Practice
- Registered General Nurse
- Experience of working within primary care
- Demonstrate continued professional development and professionalism
- Recognised qualification at degree level or above including a diagnostic
- Nurse prescribing qualification
- Strong interpersonal and communication skills and be able to liaise and communicate with clinical and administrative staff within the practice and partnership
You must have experience of providing multi-skilled healthcare and clinical treatment and be able to work with patients and members of the public. You will have experience of working on clinical systems and be able to enter and use patient data efficiently, whilst maintaining confidentiality at all times.
About Us
Walton Medical Practice is situated in the traditional seaside resort of Walton on the Naze, on the Essex coast. They have a list size of just over 8300 patients. As well as 2 GP partners and a salaried GP, the practice team includes a clinical pharmacy team of 2, a nursing team of 10 and an efficient and effective reception and administrative team of 18.
The Colte Partnership is a GP partnership, formed in June 2017. It brought together 9 local GP surgeries across Colchester and Tendring, serving approx. 95,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.
As well as general practice, we offer Extended Access appointments to enable all of our patients to access medical care at evening and weekends; we work collaboratively with external agencies to provide in home services and social prescribing; we are working to be at the forefront of clinical research; and have many more exciting ventures on the horizon.
We also work as three collaborative Primary Care Networks (PCN’s) which span across Colchester and Tendring:
- Riverside Medical Centre and Ardleigh Surgery
- Mersea Island Medical Practice, Tiptree Medical Centre and Rowhedge Medical Centre
- Walton Medical Centre, Colne Medical Centre and Wivenhoe Medical Centre
